By registering my child(ren) to participate in Chinese Community Church (CCC) 2025 Egg Hunt event, I agree to allow my child(ren)’s photo to be taken while participating. These photos may be used in craft projects, or for promotional and informational purposes including display on CCC website. Please notify CCC in writing if you object to your child(ren) being photographed.
As the Participant(s)’ parent/guardian, I accept personal responsibility for any injury or loss sustained during the Chinese Community Church (CCC) 2025 Easter Egg Hunt (the “Activity”). Additionally, I release and promise to indemnify, defend, and hold harmless CCC for any injury arising from participation in the “Activity”. If a dispute arises over this agreement or claim, I agree to resolve the matter through a mutually acceptable dispute resolution process.
